SEMI Side Effects May Include is a co-op adventure for up to four players, where the central mechanic is the use of experimental drugs with completely unpredictable consequences. Taking the pills can lead to a variety of transformations: the character can suddenly grow to gigantic proportions, gaining immense strength but losing speed, or shrink, allowing them to slip through narrow passages. Other effects cause uncontrollable tremors in the limbs, preventing interaction with objects, or grant the ability to emit electrical discharges. All this madness unfolds within the walls of a special clinic, where you control patients seeking freedom.

An additional element of chaos is introduced by a shared madness meter, which can at any moment derail control of a character, turning an ally into an unpredictable threat. For coordination, a voice chat with proximity settings allows for discussing tactics or simply observing comical situations. Each run concludes with a personalized medical report. This report humorously describes the player's playstyle and provides a humorous diagnosis reflecting their behavior during a failed or successful escape attempt.
