The Eastern Sacrifice begins with an awakening in a world devoid of incense for silent and indifferent gods. The player, as a fallen being, commands fanatics to build shelters in ancient ruins and send disciples to explore the wilds. Followers slay demons and collect treasures, helping to create a devoted religion based on your power. The goal is to reclaim the lost theurgy and achieve divine status in a world consumed by madness and chaos.

In The Eastern Sacrifice, gameplay is focused on managing the development of a domain through a sandbox building system. The process involves constructing up to 30 industrial and functional buildings, turning a desolate mountain into a powerful sanctuary. Fifteen production systems, unlocked by new edicts, automate the management of followers and supply lines. This automation ensures the accumulation of resources even outside the game, facilitating the steady expansion of territory and the growth of influence.

Adepts can be enlightened with new theurgies, turning them into zealots of the faith to expand the sect's influence. When adepts encounter fierce opponents, divine intervention allows them to inhabit their bodies and crush their enemies with their own hands. The entire gameplay leads to the Rite of Ascension - a ritual that gives huge global boosts for subsequent reincarnation. The ritual requires a heavy tribute, accepting human sacrifices and suffering, which, in turn, incurs the inexorable wrath of the ancient deities in the next cycle.
